Service Description

Home visit for evaluation and assessment, nutritional coaching, dietary lifestyle review, pantry makeover, medication organization, lab results and medical information interpretation, blood pressure, blood sugar and pulse oximetry checks, medical diagnosis review, medical discharge planning, education and personalized health/wellness plan.
